Compassionate Care for the Whole Family

Families who welcome children from hard places into their hearts and homes may experience a need for post-adoption support services. We offer a variety of Post-Permanency Services through Pennsylvania's Statewide Adoption Network (SWAN). Our services are based on the trauma-informed principles of Trust-Based Relational Intervention ® (TBRI ®), an evidence-based practice developed at Texas Christian University to meet the unique needs of children in foster care or who have been adopted.

All families who reside in the state of Pennsylvania and have adopted a child privately, internationally, or from the foster care system, as well as legal custodians and kinship parents are eligible to receive post-permanency services at no cost to families.

Our Post-Permanency Services Include

  • Case Advocacy: We provide support, resources, and assistance in accessing available services in the family's community.
  • Respite Care Services: We match adoptive families with qualified respite families who can care for children while the parents get a night out or a weekend away. Other types of respite opportunities are available based on the specific needs of the family.
  • Support Groups: We connect families with a support system of peers who can relate to the challenges of parenting, provide encouragement, spiritual support, and social opportunities.
  • The HALO Project: This evidence- based 10-week intensive outpatient treatment program is for families struggling with the impacts of childhood trauma. Contact us to determine if your family qualifies for support through SWAN Post Permanency Services. For program information, go to: www.SalvationArmyHalo.org

How to Request Post-Permanency Services:

Families interested in learning more about receiving Post-Permanency Services should contact SWAN at 1-800-585-
SWAN
. A trained staff member will discuss your needs and work with you to determine which services would be most
helpful to your family.

Families can request The Salvation Army Foster Care & Adoption Program to provide service to your family. The SWAN
representative will contact us and make a referral on your family's behalf. Once we receive the referral, a case worker will
contact you to discuss next steps.
