Family Foundations Program

This community-centered initiative supports vulnerable families in the Lehigh Valley, delivering trauma-informed services to help families heal and thrive.

Overview

The Family Foundations Program is a community-centered initiative that supports vulnerable families in the Lehigh Valley by addressing three deeply interconnected challenges: economic hardship, housing insecurity, and unresolved trauma.

Grounded in evidence-based practices and guided by Trust-Based Relational Intervention® (TBRI®), we deliver bilingual, trauma-informed, wraparound services to help families heal, grow, and thrive.

Our Mission

We believe that when families are supported, communities flourish. Our mission is to break cycles of instability—including homelessness, recidivism, and youth violence—by providing families with the tools, resources, and relationships they need to create safe, nurturing homes and lasting stability.

We help caregivers build stronger relationships and environments through:

- Parenting support

-Skill-building workshops

- Communication and connection tools rooted in trauma-informed practices

When families are empowered, children are more likely to succeed at home, in school, and in their communities.

We work to address financial and housing insecurity by providing:

- Personalized case management

- Access to job training and employment services

- Public benefits navigation

- Affordable housing resources

A stable home and steady income are critical to long-term family well-being.

Many families have experienced deep trauma - violence, poverty, displacement.  Our response includes:

  • Trauma assessments and mental health support

  • Trust-Based Relational Intervention® (TBRI®) services

  • Group-based healing programs like Family Nurture Groups and Making Sense of Your Worth (MSOYW) and Parenting for Positive Self Worth (PPSW)

  • In-home trauma-informed interventions

We help families not only survive, but heal and grow stronger.

Our work doesn’t just address symptoms—it goes deeper. Using a holistic, two-generational approach, we target the root causes of instability. We support both children and their caregivers, recognizing that true transformation comes when the whole family is lifted up.

Primary Care Communities: Strengthening Families Through Support

Our Family Foundations Program is expanding to include Primary Care Communities (PCCs) — volunteer-led support networks that walk alongside families experiencing challenges. Whether a family is facing housing instability, parenting alone, reuniting with their child after foster care, or simply lacking a reliable support system, a Primary Care Community can make all the difference.

What is a Primary Care Community?
A PCC is a team of trained volunteers who come together to surround a family with practical, emotional, and spiritual support. They might bring weekly meals, celebrate birthdays and holidays, offer a listening ear, and help connect the family to local resources. These communities promote healing, stability, and resilience — often preventing the need for foster care altogether.

How it Helps
Families served by a PCC report feeling less isolated and more equipped to meet their children’s needs. Volunteers commit to walk with a family for 6 to 24 months, forming bonds that can lead to lifelong relationships. PCCs serve as a living example of what it means to be "the village" every family needs — helping parents succeed and children thrive.

Get Involved
We believe that families do better when they’re surrounded by care, and you can be part of that change. If you’re interested in serving in a Primary Care Community, or if your church or organization would like to help launch a PCC, we’d love to hear from you!
